Special Graduation to be held in September at the YMCA
In September, around 40 patients will be graduating from the Diabetes Self-Management Program - Amigos en Salud/Friends in Health. Graduation is schedule at the YMCA where CHM and YMCA officials will present a diploma to CHM patients to acknowledge this accomplishment. Also this event will celebrate a one year and of partnership between YMCA and CHM.
