Community Health Mission measures its achievements in numbers, but true measure of success is in the reduction of suffering and improvement of individual health. Taken together, the individual improvement reflect a significant impact on the health and quality of life in our community. In terms of value, primary healthcare services provided by CHM to the community is estimated at $500 for every emergency diversion.

o A cultural appropriate women’s health program in partnership with the NBLIC-III funded by the American Cancer Society
o Self-management diabetes and cardiovascular program: Amigos en Salud / Friends in Health program that assists Hispanic/African American patients and their family members to understand and self-manage diabetes and co-morbid depression through trained Community Health Workers (CHWs).
Ø Prescription Program (Patient Assistance Program for long term drug treatment). This program assist the uninsured in obtaining prescription medications free-of-charge from pharmaceutical companies. Once patient is approved by the pharmaceutical company he/she Patient will get a 90 days supply of medication. This program is conducted in partnership with MedBank, Inc
Ø Insulin and calibrated syringes for diabetic patients for those who are insulin dependent
Ø Glucometer and 100 diabetic strips during first visit for diabetic patients.
Ø Health information and awareness of our services; bilingual newspaper articles about health issues.
Ø Translation services for all patients (English into Spanish) 5 days a week during clinic ours
Ø Two convenient hours of operation Monday-Friday 8:30 am-3: 30 pm and Thursday from 4:30 pm – 8:00 pm.
