Eligibility Criteria

  1. Adults age 18-64 years of age
  2. Patients must be uninsured (No Medicaid , no Medicare, and no private or employer health insurance
Documents needed

• Proof of income, (letter from employer, tax forms, W-2, etc).  If patient is not working, patient is required to bring a letter of the person responsible for his/her expenses.  If pay in cash, a patient will be required to sign a document acknowledging that the amount he is declaring is true.

• Copy of a picture ID (license, passport, consulate ID)

• Proof of place of residency (electricity bill, etc)

  1. Monday - Friday 8:30am – 3:30pm, Tuesday andThursday 5:00pm -8:00pm

Women's Health Clinic

  1. Monday, Tuesday, Wednesday, and Friday 8:30AM - 12:00 AM
